Recipient Nomination
This is an annual fundraiser to help those in our community who are suffering from the effects of medical crisis, personally or within their families. We raise funds directly for the family, with the help and support of our town.
To nominate someone, please send an email request, up to one page describing the person and their affliction.
Please describe their Carver residency or their affiliation with Carver.
Please describe their medical issue.
Please let us know how this has affected the person/family to create the need for assistance.

*Please note: You may not nominate yourself. The chosen recipient(s) will need to consent to the Carver Lions Club using their name and medical issue in advertising for the fundraiser event.
